What Is a DOT SAP Evaluation?

If you've tested positive for drugs or alcohol — or refused a DOT-mandated test — federal law requires you to complete a Substance Abuse Professional evaluation before returning to any safety-sensitive position. This includes commercial truck driving, aviation, rail, transit, pipeline, and maritime roles.

Who is required to complete a DOT SAP evaluation?
Any employee in a DOT-regulated safety-sensitive position who tests positive, refuses a test, or otherwise violates DOT drug & alcohol policies — including CDL drivers, bus operators, aviation workers, pipeline employees, and railroad workers.
What is the Federal Drug & Alcohol Clearinghouse and do I need to register?
Yes — registration is required. The Federal Drug & Alcohol Clearinghouse is an FMCSA database that tracks DOT drug and alcohol violations for commercial drivers. After a positive test, refusal, or other violation, you or your employer must report it to the Clearinghouse before you can begin the SAP process. Register at clearinghouse.fmcsa.dot.gov.
Can I return to work before the process is complete?
No. Federal law prohibits employers from allowing a safety-sensitive employee to return to duty until the full SAP process — initial evaluation, recommended program, and follow-up evaluation — is completed and RTD clearance is issued.
How long does the entire SAP process take?
The initial evaluation can typically be scheduled the same week you call. After completing your recommended program (ranging from a few days to several weeks), you'll schedule a follow-up evaluation with Bonnie to receive your RTD clearance.
What does the SAP evaluation cost?
The initial SAP evaluation fee is $400.00, paid out-of-pocket at the time of your appointment. SAP services are not covered by insurance. This fee covers your comprehensive evaluation and written recommendation. Follow-up evaluation fees will be discussed at your initial appointment.
Is what I share with the SAP kept confidential?
Yes. Your evaluation is confidential under HIPAA and DOT regulations. The only information provided to your employer or MRO is whether you are eligible to return to duty — not clinical details from your sessions.
